Well-being Checks Faulted in Teen’s Death at Red Wing Juvenile Center

A state investigation has uncovered multiple violations at the Red Wing juvenile correctional facility leading up to the death of a 16-year-old last fall, according to a report by the Star Tribune.

The violations included failures in conducting proper well-being checks on the night the juvenile was found unresponsive. The Star Tribune, citing a Department of Corrections investigation, highlighted these lapses in oversight.

Additionally, the report noted that the facility administered an unspecified medication to the juvenile without obtaining consent from a parent or guardian.

The teenager was discovered deceased on October 19, 2024. The Southern Minnesota Regional Medical Examiner’s Office has ruled the death a suicide, the Star Tribune reports.
